AgeWell is a peer-to-peer care model is designed to improve well-being and promote health outcomes among older adults. In the pilot, AgeWelll will test its model with 30 of Henry Street Settlement’s clients in order to reduce hospitalizations and emergency room visits.
Bluestream Health / Jewish Home Lifecare
Bluestream Health brings remote expertise into the context of care delivery. This Pilot implements the Bluestream Platform at Jewish Home’s 106th St. Skilled Nursing Facility, bringing remote expertise to the bedside to prevent avoidable readmissions and ER visits.
Blue Mesa Health / Mt. Sinai Hospital
Blue Mesa Health has developed a platform to help prediabetic patients lose weight through technology and health coaching. In their pilot, Blue Mesa will distribute their platform, connected scales and activity trackers to 200 Mt. Sinai employees to reduce their risk for developing type 2 diabetes.
InGen (Innervation + Genioglossus) / Weill Cornell Medicine Center for Sleep Medicine
InGen (Innervation + Genioglossus) and the Weill Cornell Medical College’s Center for Sleep Medicine have partnered to conduct a pilot study of targeted genioglossal strengthening on obstructive sleep apnea. The innovation is a promising alternative to existing treatments that many patients find uncomfortable and difficult to comply with.
Quality Reviews, Inc. / Mount Sinai Health System
Quality Reviews’ product, RateMyHospital®, is a HIPAA-secure, patient relationship management platform that enables healthcare provider organizations to measure, manage, and improve the patient experience in real-time via mobile devices. Quality Reviews is piloting the system with Mt. Sinai and will track patient satisfaction as the host implements service improvements at 2 facilities.
RecoverLink / Mount Sinai Hospital
Using RecoverLink’s advanced algorithms, Mount Sinai hospital will be able to “virtually observe” heart failure patients at their homes in order to identify negative health behaviors, provide early intervention when necessary and course-correct risky behaviors before a readmission occurs. This pilot will determine the cost-savings, efficiency and overall satisfaction with RecoverLink versus the current standard of care.
RubiconMD / Callen-Lorde Community Health Center
RubiconMD is a web-based platform that allows primary care providers to send patient cases to specialists for quick e-consults. In the pilot, all Callen-Lorde providers will have access to RubiconMD to submit unlimited consults to top specialists when they need it. RubiconMD will collect data on the impact of these interactions and work with Callen-Lorde to quantify the impact on improved clinical outcome and avoided unnecessary costs and wait times.
SensaRx, LLC / Hebrew Home at Riverdale
SensaRx’s SafeWander Button Sensor is a secure way to alert caregivers when their patients rise from their beds thus, enabling them to provide a timely response and to monitor their patients. Their pilot with Hebrew Home at Riverdale will demonstrate the effectiveness of the SafeWander Button sensor in preventing residents’ falls and reducing response time to falls at night.
TweeQ LLC / VillageCare
TweeQ combines an easy-to-use patient app that personalizes and motivates home exercise, with a platform that allows therapists to track compliance and gather feedback. In light of new reforms to bundle payments physical therapy, TweeQ and VillageCare will test the ability of their technology to maximize quality outcomes for improving functional gains while minimizing cost to Medicare.
Yosi, Inc. / Healthfirst
Yosi is a cloud-based mobile patient check-in and communication platform that helps patients manage their care across various providers, improving patient experience, access to preventive healthcare and provider workflow efficiencies.

Smart Vision Labs / SUNY College of Optometry
Smart Vision Labs wants to let anyone in the world test their vision and get a prescription by taking a picture of their eye with a smartphone. Smart Vision Labs and the SUNY College of Optometry in New York City will compare this new vision test using Smart Vision Lab’s smartphone attachment against standard tests that require machinery costing $10,000+. The company aims to disrupt the entire optical industry and revolutionize the way people get their eyeglasses worldwide.
QoL Devices, Inc. / Montefiore Medical Center
QoL Devices and the Children’s Hospital at Montefiore will pilot Alv.io, an advanced, mobile-connected respiratory training and monitoring device that uses interactive, animated games to incentivize engagement, personalize training, conduct performance analysis and monitor testing in children to improve asthma management. In an effort to improve asthma care and reduce costs, the device will be piloted with 100 asthmatic children who seek treatment with Montefiore in the Bronx.
Urgent Software / Mount Sinai Health System
Urgent Software will work with Mt. Sinai Health System’s Cardiovascular Institute (CVI) cardiologists and their out-of-network referring physicians. As part of Mt. Sinai’s community outreach to neighborhoods at high risk of cardiac disease, CVI physicians visit local physicians’ offices on a daily or weekly basis to provide cardiac consultations to patients. Though CVI physicians provide excellent care, patients often choose not to follow up on referrals to cardiac specialists when the patient’s insurance doesn't cover the recommended procedure or specialist. The Urgent Consult platform will allow local physicians and admin staff to search for a nearby CVI physician who accepts the patient’s insurance, schedule an appointment, share medical info independent of EMRs, and register the patient, collecting demographic and insurance info. The pilot will thus test the platform’s ability to help local primary care physicians effectively link their patients to life-saving specialty care.
Nonnatech / ElderServe
Nonnatech and senior care provider ElderServe will focus on the early detection and intervention of physiological changes in an effort to prevent hospitalizations. By using Nonnatech’s system of smart sensor technologies, the pilot will measure seniors’ medication adherence and monitor activities such as eating, fluid intake, sleeping, toileting, and socialization patterns. This is all done while allowing the patients and residents to retain their dignity and privacy. Looking at the cost across the continuum of care, it will also create cost savings by way of more efficient use of staff in the Managed Long-Term Care Program and in the Assisted Living environment.
Fit4D and Healthfirst, which provides free and low-cost health insurance, are partnering to keep diabetes sufferers in the Bronx healthy and out of the hospital. Fit4D’s certified diabetes educators will use powerful algorithms to match Healthfirst members with the best diabetes treatments for each individual. The goal of the project is to test whether Fit4D can keep these members from suffering complications that will drive up healthcare costs.
AllazoHealth / Accountable Care Coalition of Greater New York
ACCGNY will implement a medication adherence program powered by AllazoEngine’s predictive analytics, which reviews patients’ claims, demographic and intervention data to anticipate the need for intervention and identify the most cost-effective method. In a joint pilot, the team will focus on ACCGNY’s 6,353 attributed beneficiaries, 63% of whom are intellectually or developmentally disabled (IDD). Despite their low medication adherence rates and high costs, IDD patients have largely been overlooked by medication adherence programs and academic research, making this pilot critically important. The pilot will evaluate the accuracy of the predictions, the impact of interventions and the savings associated with adherence.
Canopy Apps / Visiting Nurse Service of New York
Canopy Apps and VNSNY will seek to improve communications with non-English speaking patients using the Canopy Medical Translator app. The app will enable VNSNY nurses and physical therapists to access a library of pre-translated medical phrases in 15 languages to communicate with patients during home visits. The pilot will seek to measure patient and provider satisfaction with patient-provider communication, as well as increased efficiency in the VNSNY workflow.
Healthify / VillageCare
The Heathify platform screens patients for their needs, connects them to the appropriate resources for treatment, and engages them through text messages. Sixty percent of a person’s health is affected by social issues like food insecurity, unsafe housing, domestic abuse and other issues. VillageCare’s Health Home case managers will utilize the Healthify platform to connect patients with the services they need to address these “social determinants of health.” Moreover, VillageCare case managers will utilize the Healthify resource database to rate and review different community and government resources so VillageCare can direct their patients to the best possible help.
Tactonic Technologies / NYU Langone, Rusk Rehab Center
Tactonic Technologies is partnering with NYU to test whether its system of imaging sensors and powerful software can help doctors at the NYU Rusk Center for Rehabilitation measure whether a patient is ready to leave the hospital. Doctors check to see whether elderly patients can walk steadily before discharging them but often make a subjective judgment call that can lead to unnecessary slips and falls that bring the patient back to the emergency room. Tactonic will allow NYU doctors to make an objective measurement of patients’ gait.

AdhereTech / Weill Cornell Medical Center
AdhereTech has created wireless smart pill bottles that improve medication adherence. Dr. Roy Gulick is a Professor of Medicine at Weill Cornell and the Principal Investigator of the Cornell HIV Clinical Trials Unit. AdhereTech will be working with Dr. Gulick and his team to assess patient adherence to an HIV drug regimen (tenofovir-based) over 12 weeks using AdhereTech bottles with reminders and feedback, compared to the usual standard of care.
BioDigital / Memorial Sloan-Kettering Cancer Center
BioDigital and Memorial Sloan-Kettering are addressing deficiencies in patient-doctor communication through the use of new visualization technology. BioDigital's Human Platform, an interactive 3D virtual body that is already widely used in academia, will be introduced into the Memorial Sloan-Kettering clinical workflow as a tool to more effectively communicate complex health topics. The expected results include increased patient engagement, understanding and satisfaction - all while saving doctors time.
Bio-Signal Group / New York Methodist Hospital
Each year, in the USA alone, over 12M patients present to emergency departments in an altered mental state, in need of an electroencephalogram, but cannot get one. microEEG is an FDA approved, miniature, wireless system that utilizes a rapid-to-apply disposable electrodes and a network of teleneurologists to provide a remote, rapid interpretation to an ED physician. Utilizing microEEG in the ED can influence patient management and thus alter economic outcomes.
eCaring / Pace University
This pilot will test the outcomes of the eCaring web-based care management & monitoring system with a high need population of chronically ill, multicultural older adults in Henry Street Settlement’s Vladeck Cares Naturally Occurring Retirement Community (NORC). Patients & their caregivers will be randomly selected to receive eCaring for 6 months. Pace RN’s will track data & assess the effect on use of healthcare resources, disease management, preventable events, & costs.
Opticology / New York Eye & Ear Infirmary (Now Mount Sinai)
Diabetics must measure their blood glucose despite being painful and inconvenient. We have developed a better way to measure blood glucose in the 17 million Type II diabetics: through ocular tears. Our technology consists of a pen shaped device with glucose sensing chemistry at the tip that gently touches the tears just above the lower eyelid. Our pilot will measure the correlation between tear and blood glucose in diabetic patients.
Sense Health / University Behavioral Associates (Affiliated with Montefiore)
Sense Health is working with University Behavioral Associates (affiliated with Montefiore) to pilot their mobile patient support technology with case managers and patients in the Managed Addiction Treatment Services Program. If successful, this pilot will validate a technology that efficiently helps case managers deliver high intensity support to growing patient case loads.
StarlingHealth / VillageCare
StarlingHealth and VillageCare are partnering to pilot the use of a nurse call and bedside communication system to improve patient care and satisfaction while reducing cost of care delivery at VillageCare Center for Rehabilitation and Nursing, a 105-bed sub-acute facility. Residents will use multi-lingual touchscreen tablets to make specific requests. Automatic request delegation, routing and escalation will help staff resolve needs, and management will have real-time visibility into all activities.
Vital Care Services / Pace University
Vital Care Services and Pace University will pilot the effectiveness of TeleHealth solutions to meet current and projected needs of diverse socio-economic communities in New York City. Pace student technicians will assist patients in taking their vitals, which are monitored remotely by a Pace RN. Key objectives of the pilot are to reduce healthcare cost, increase access and quality of care, and train future working professionals at the University level.
